Community Health Worker - Certificate of Achievement

The Community Health Worker (CHW) Associate Degree provides the opportunity for the student to develop skills as frontline public health worker who is a trusted member of and/or close understanding of the community they serve. This relationship enables the CHW to serve as a liaison between health services and the community. Course requirements for the major can be completed in two or three semesters of study. Classroom theory is applied in a CHW Internship course with placement of the student to practice skills learned in a variety of healthcare settings. Students must maintain a grade of C or better in all core classes and program electives to satisfy degree requirements.

Program Learning Outcomes:
  • Students will be able to identify the personal qualities, core competencies relating to work-setting skills, knowledge base, health promotion/education, and service coordination, researching and health coaching.
  • Students will know the current community health care issues and community agencies and resources available for consumers today.
  • Students will be able to effectively advocate for environmental safety issues while working with clients of diverse ages, cultural back grounds, intellectual and emotional abilities.

To earn this certificate, students must complete the minimum required courses with a grade of C (or P) or better.

A community health worker builds health understanding and self-sufficiency in individuals within the community through a range of activities such as: outreach, community education, informal counseling, social support and advocacy.
